A few of our treatments need a health check first — not to catch you out, but because things like medication, pregnancy and recent sun genuinely change what's safe. Filling this in before you arrive saves time in the room.
It takes about three minutes. Answering “yes” to something rarely
rules a treatment out — it just tells us what to adjust, or what to check with you first.
Laser Hair Removal
Laser and IPL. Includes skin type, sun exposure and medication screening.
Skin Treatments
Microneedling, exosomes, chemical peels and the Oxy Facial.
Massage & Bodywork
All massage, including lymphatic drainage and prenatal.
Waxing, Threading & Tinting
Short form. Tinting requires a patch test 24 hours ahead.
Your privacy. These forms go directly to Evergreen Essence and nowhere else.
The information is used only to keep your treatment safe and to keep a record of your care.
It is never sold or shared, and you can ask to see, correct or delete it at any time by emailing
